In neurointensive patients with acute lung injury, achieving the goal of lung protection without threatening cerebral perfusion is very difficult. In patients with more refractory raised intracranial pressure, the optimal balance between brain and lung may not be well established. Multiple strategies are used to recruit atelectatic alveoli and improve oxygenation.
