| 50 Chapter 5 History of Total Ankle Arthroplasty culations that function together to reduce axial and shear constraint at the bone-implant interfaces. Theoretically to reduce constraint fixed-bearing ankles can be only partially conforming. As a result they have higher wear rates because polyethylene contact stresses are increased. With less conformity wear is typically greater. This is particularly true when the prosthesis is not perfectly balanced. In such a case edge loading may occur leading to high contact stress on thin polyethylene. The two-component TNK and aGiLITY implants have fixed bearings. They decrease stress at the bone-implant interface by being less conforming than first-generation designs. For example - The TNK design attempts to solve fixation difficulties by means of hydroxyapatite-coated ceramic and a tibial component screw. - The agility design includes a syndesmotic fusion to prevent tibial component subsidence and also resurfaces the medial and lateral ankle to enhance fixation and alignment. Theoretically wear in these partially conforming articulations manifests primarily as delamination and pitting with some secondary abrasion. Three-component implants such as S.T.A.R. Buechel-PappasTM HINTEGrA and SALTO have mobile bearings with full conformity and minimal constraint. They are designed to reduce load transfer to the bone-implant interface and to decrease polyethylene contact stress while maintaining ankle kinematics. All of these designs require only minimal bone resection and simply resurface the talus. Theoretically wear in these fully conforming articulations occurs primarily by abrasion. 5.3.2 Two-Component Designs The TNK design Nara Japan 116 is a fixed-bearing prosthesis and since 1980 has been made of ceramic and employed without cement 116 117 . Bony fixation methods include hydroxyapatite-coated beads and a tibial screw. The ceramic concave talus articulates with a flat polyethylene tray that includes a flange to resurface the
