He had four or five previous episodes of palpitations in the past but they had lasted only a few minutes and went away on their own. These episodes were not associated with any specific activity or diet. He denies any chest pain. On physical examination he is noted to be tachycardic with a heart rate of 180 bpm and a blood pressure of 105 70 mm Hg. An ECG shows a wide complex tachycardia at 180 bpm. The tachycardia terminates suddenly and the patient s heart rate drops to 90 bpm. A repeat ECG shows sinus rhythm with a short PR interval and a wide QRS with a slurred upstroke delta wave .
