Propofol is very lipid soluble, has a large volume of distribution, and can be given for prolonged periods of time without significant changes in its pharmacokinetic profile. Because propofol has no active metabolites, the termination of its clinical effect is dependent solely on redistribution to peripheral fat tissue stores. When the infusion is discontinued, the fat tissue stores redistribute. Because of its pharmacokinetics and specific effects on cerebral hemodynamic variables, preserving autoregulation and vasoreactivity to carbon dioxide, propofol approximates the ideal sedative. Intravenous bolus administration produces a dose-dependent, coupled decrease in CBF and CMRO2 similar to that described after barbiturate administration.
