You are considering concentrating some critical care services into high-volume regional centres of excellence as has been done in other fields of medicine. In your region this would require several centres with differing levels of expertise that are geographically separated. Given there are inherent risks and time delays associated with interfacility patient transport you debate whether these potential risks outweigh the benefits of regional centres of excellence. Introduction Critical care is a highly complex expensive and resourceintensive dimension of the healthcare system 1 and the demand for these services is expected to grow due to the aging population 2 3 . Regionalization of critical care services has received much attention as a strategy to improve patient outcomes and to realize efficiencies in care delivery 4-7 . Regionalization entails the allocation of scarce healthcare resources on the basis of geography and has been implemented in other areas of medicine including trauma .
